We also believe that stakeholders must share in the responsibility of encouraging, motivating and assisting consumers to accept responsibility for their behavior. Utilizing the most practical and systematic approach, services should be customized to compliment and strengthen individuals.

1. Age 18 and over
2. The consumer should exhibit significant functional limitations that prohibit them from maintaining community stability and independence in the most appropriate, least restrictive environment.
3. The consumer must be willing to participate in the services/programs.
4. The consumer shall have prior history of the following: psychiatric hospitalization, residential crisis stability, Intensive Community Treatment (ICT), Program of Assertive Community Treatment (PACT), psychiatric residential facility (RTC Level C), or Temporary Detention Order (TDO). This criterion shall be met in order to be initially admitted to services.

QUALIFIED MENTAL HEALTH PROFESSIONAL (QMHP)
Individual(s) with at least a Bachelor's degree in Human Services or related field (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling or other degree deemed equivalent) from an accredited college and with at least one year of clinical experience providing direct services to individuals with a diagnosis of mental illness.
